A look into the new nasal anatomy concept by José Carlos Neves — the Fusion Sling. In this video, you will first see the key structure — the Perichondrium–Periosteum Blanket: a continuous “cover” that, at the junction of the upper and lower lateral cartilages, forms a dense anatomical unit called the Fusion Sling. This blanket provides structural support and serves as the foundation surgeons can use for tip stabilization and for maintaining tip position in modern rhinoplasty.

From there, the video explores the Fusion Sling in greater detail, followed by other crucial anatomical discoveries:
– Vertical and Horizontal Densifications — consolidated zones that redefine previous interpretations of the Pitanguy ligament
– The SOON Muscle (Superficial Orbicularis Oris Nasalis Muscle), located just anterior to the Vertical Densification and the medial crura of the lower lateral cartilages
– Unique fat compartments — one in the supratip area and another between the medial crura and the septum. These two fat pads are completely separate and do not connect.

Historically, nasal anatomy in rhinoplasty — particularly in aesthetic rhinoplasty — was focused mainly on ligaments and ligamentous structures. The Fusion Sling shifts this perspective, highlighting the importance of tissue density, consolidation, structural integrity, and stabilization in nasal surgery.

These findings, published by Portuguese facial plastic surgeon José Carlos Neves in his PubMed article Fusion Sling, open entirely new perspectives for rhinoplasty planning and redefine our understanding of nasal anatomy.
