Doctor Samuel Sterns is a cellular biologist and Grayburn College professor who worked with Bruce Banner in attempt to find a cure for Banner. Sterns managed to engineer a temporary antidote for Banner's condition but was also forced to administer Banner's blood to Emil Blonsky, transforming him into Abomination. When a drop of Banner's blood fell into an open wound on Sterns' head, his brain suffered a rapid mutation, greatly increasing his mental abilities. However, Natasha Romanoff arrived and subdued him, handing him into S.H.I.E.L.D.'s custody.
Sterns was officially blamed for Abomination's creation and the subsequent Duel of Harlem by General Thaddeus Ross, who imprisoned Sterns at Camp Echo One. There, Ross exposed Sterns to more gamma radiation to further amplify his intellectual capacities and forced Sterns to create technology for the government as well as heart pills for Ross. Sterns was eventually offered a release and pardon by Ross if he helped Ross become President of the United States, which Sterns agreed to. However, when Ross refused to honor the deal upon becoming President, Sterns became the master manipulator Leader and enacted his revenge by orchestrating a plot to destroy Ross' legacy. Using a mind control device that made soldiers, including the super soldier Isaiah Bradley, do his bidding, Sterns attempted to trigger a world war and ultimately caused Ross to transform into Red Hulk. When his plans were foiled by Sam Wilson, Sterns surrendered and was then imprisoned in the Raft, where he warned Wilson of Multiversal threats to come.
Ross kept Sterns as his prisoner for sixteen years, using Sterns' incredible intellect to come up with technology and weapons for the United States. Sterns also assisted Ross to climb the political ladder, from Secretary of State for President Matthew Ellis, to running for the Presidency himself. Sterns agreed to go along with helping Ross, being promised a full pardon for his crimes that he had been blamed for. But years of being left to rot as a prisoner led to Sterns deducing that Ross would not keep his word, but would continue manipulating him for his political gains.[4]

In 2027, he sends Sam Wilson and Lieutenant Joaquín Torres, known by their recently acquired mantles Captain America and Falcon respectively, on a mission in Oaxaca, Mexico to stop SERPENT's illegal sale of adamantium. Ordering Navy SEALs to secure the metal, Captain America storms the Catedral de Santo Ramos to rescue hostages from SERPENT leader Seth Voelker, codenamed Sidewinder, who urged them to reveal the organization's buyer's whereabouts. Falcon intercepts SERPENT's escape attempt and retrieves the stolen canister containing adamantium while Captain America faces SERPENT member Copperhead.
