If you’re planning to change your name after marriage, or even if you're unsure about changing your name after marriage, it’s important to include your prospective name change at the bottom of your marriage license. That doesn't cause your name to change. It just leaves that option available to you so that in the future you can take a certified copy of your marriage license and certificate to the Social Security Administration, the Department of Motor Vehicles, and the Passport Office and they'll permit you to change your name but only exactly as it appears at the bottom of your marriage license document. So even if there's only a tiny chance that you'll be changing your name, you should include that name change at the bottom of the marriage license document. Because if you do choose to change your name and it’s not included at the bottom of the marriage license document then the only way to do so is to file a petition in Superior Court for name change. And that can cost you hundreds of dollars and much more time than you ever wanted spend in court."
