### Why These Tests Matter: When I evaluate a patient for lower eyelid surgery, it’s crucial to assess the eyelid’s tone and support because these factors can predict potential issues like ectropion (where the eyelid pulls away from the eye), lid retraction, or too much white of the eye showing (scleral show). If the lower eyelid lacks good structure, it’s more prone to these complications after surgery. These tests also guide how I plan the surgery. For example, if I notice significant laxity, I might recommend adding a lateral canthopexy to tighten and support the eyelid. On the other hand, if there’s borderline tone, I’ll approach skin and fat removal more conservatively to avoid creating unnecessary tension on the lid. Finally, these tests are invaluable for discussing outcomes with patients.
