The last chord of the sequence is interpreted as a dominant and then leads to the new key. Each key change shifts the chord progression up a minor third. After this has happened four times, you get back to the original key.
Cm - Ab - Eb - Bb (Bb is the dominant of Eb major or Eb minor - but we use D# as the new key)
D#m - B - F# - C# (C# is the dominant of F# major or F# minor)
F#m - D - A - E (E is the dominant of A)
Am - F - C - G (and with G as the dominant you get back to C minor, which is where it all started.)
